1. PARENT OR LEGAL GUARDIAN AUTHORIZATION

I, {{guardian_full_name}}, represent that I am the parent or legal guardian of {{participant_full_name}} (“Minor Participant”) and have legal authority to enter into this agreement on the Minor Participant’s behalf.

I voluntarily request and authorize the Minor Participant to participate in {{event_name}}, including the {{race_option_name}}, and any associated race, run, walk, relay, training activity, athletic competition, or related activity (“Event”).

2. ACKNOWLEDGMENT AND ASSUMPTION OF RISK

I understand that participation in the Event involves inherent and other risks, including but not limited to running or walking on streets, sidewalks, trails, tracks, parking areas, uneven surfaces, hills, curbs, intersections, and other courses; contact with vehicles, bicycles, other participants, spectators, equipment, animals, or fixed objects; slips, trips, falls, collisions, weather conditions, heat, cold, dehydration, overexertion, illness, and other physical or environmental hazards.

I understand that participation may result in property damage, illness, serious bodily injury, permanent disability, or death.

On behalf of myself and the Minor Participant, I knowingly and voluntarily accept and assume the risks associated with participation in the Event, whether known or unknown, foreseeable or unforeseeable, except to the extent such risks cannot legally be waived under applicable law.

3. MINOR PARTICIPANT FITNESS AND ABILITY

I represent that {{participant_full_name}} is physically and medically able to participate in the Event and that I am not aware of any condition that would make participation unsafe.

I understand that it is my responsibility to determine whether the Minor Participant should participate and to obtain appropriate medical advice when necessary.

I agree that the Minor Participant will follow Event rules, course directions, safety requirements, and instructions provided by Event officials, volunteers, law enforcement personnel, medical personnel, or other authorized persons.

4. RELEASE AND WAIVER OF LIABILITY

To the fullest extent permitted by law, on behalf of myself, the Minor Participant, and our respective heirs, representatives, guardians, successors, and assigns, I release and agree not to sue the Los Angeles Police Revolver and Athletic Club (LAPRAAC), its officers, directors, employees, members, volunteers, agents, sponsors, affiliates, contractors, timing personnel, race officials, property owners, governmental agencies, and other persons or entities involved in organizing, sponsoring, conducting, or supporting the Event (collectively, the “Released Parties”) for claims arising out of or relating to the Minor Participant’s participation in the Event.

This release includes claims arising from the ordinary negligence of the Released Parties to the fullest extent permitted by applicable law.

This release does not apply to conduct that cannot legally be released or waived.

5. PARENT/GUARDIAN AGREEMENT AND INDEMNIFICATION

I understand that I am signing this agreement both individually and as the parent or legal guardian of {{participant_full_name}}.

To the fullest extent permitted by law, I agree to indemnify and hold harmless the Released Parties from claims, demands, liabilities, damages, or expenses arising from the Minor Participant’s participation in the Event or from any claim brought by or on behalf of the Minor Participant, except to the extent prohibited by law.

6. EMERGENCY MEDICAL AUTHORIZATION

If the Minor Participant becomes ill or injured during the Event and I cannot be reached, I authorize Event personnel, volunteers, emergency medical personnel, physicians, hospitals, and other health-care providers to provide or arrange reasonably necessary emergency medical evaluation and treatment.

I authorize emergency transportation when reasonably necessary.

I understand that the Released Parties are not responsible for the cost of medical treatment, transportation, hospitalization, or related services.

7. EVENT RULES AND REMOVAL FROM PARTICIPATION

I understand that Event officials may remove or disqualify the Minor Participant from participation when they reasonably believe removal is necessary for safety, rule enforcement, course management, or proper operation of the Event.

I understand that removal or disqualification does not necessarily entitle the participant to a refund.

8. PHOTOGRAPHY, VIDEO, AUDIO AND MEDIA RELEASE

I authorize LAPRAAC and persons acting on its behalf to photograph, video record, audio record, livestream, reproduce, publish, display, distribute, and otherwise use the Minor Participant’s name, image, likeness, voice, race participation, and Event results in connection with the Event and LAPRAAC activities.

Such use may include websites, social media, newsletters, promotional materials, publications, historical archives, broadcasts, and similar communications.

I understand that photographs or recordings may contain groups of participants and that it may not be practical to identify or exclude a particular participant from photographs or recordings made during a public athletic event.

I understand that no compensation will be provided for such use.

This media authorization is a required condition of participation in the Event.

9. RACE RESULTS AND PARTICIPANT INFORMATION

I understand and agree that Event-related information regarding {{participant_full_name}} may be publicly displayed or published as part of race administration and results.

This information may include the participant’s name, bib number, age or age category, gender/category when applicable, team, race option, finishing time, placement, penalties or result status, and other information reasonably necessary to administer and publish Event results.

10. UNDERSTANDING AND VOLUNTARY AGREEMENT

I acknowledge that I have carefully read this entire agreement and understand its contents.

I understand that I am giving up substantial legal rights on behalf of myself and, to the extent permitted by law, the Minor Participant.

I represent that I am the Minor Participant’s parent or legal guardian and that I have authority to sign this agreement.

I enter into this agreement knowingly and voluntarily.
